Privacy policy

What this website collects, why, and how to make us delete it.

This policy covers plaskerchiropractic.com — the website. Your clinical record is governed separately by our Notice of Privacy Practices under HIPAA, which is the stricter of the two. Where they overlap, the Notice wins.

What we collect, and only when you hand it over

Appointment requests: your name, phone, email and whatever you tell us about why you are coming in. We need this to call you back and to prepare for the visit.

Interactive tools: the answers you give are processed in your browser to produce your result. Emailing that result to yourself is optional — the tool works fully without an email address, and if you do not enter one we have no way to identify you and keep nothing that could.

Downloads and guides: an email address, so we can send the file and, if you asked for it, follow-up articles.

Nothing on this site asks for a Social Security number, insurance ID, payment card or clinical history. If a page ever appears to, it is not ours — close it and call us.

What is collected automatically

Standard web analytics: pages viewed, approximate location from your IP address, device and browser, and how you arrived. We use Google Analytics for this. It tells us which pages are useful; it does not tell us who you are.

We do not sell, rent or trade your information. We do not share it with advertisers. We do not run advertising pixels that build a profile of you across other websites.

SMS consent and data sharing

We highly value your privacy. Your personal information, including mobile numbers and SMS consent, will never be shared with or sold to third parties or affiliates for marketing or promotional purposes.

If you have opted in to receiving text messages from us, we will use your mobile number solely for the purpose of communicating with you regarding your service appointments, quotes, essential account updates, and occasional promotional messages.

Email, and how to stop it

If you asked for articles or guides, every email we send carries a one-click unsubscribe link and a preference link that lets you narrow what you receive rather than leave entirely. Unsubscribing takes effect immediately and does not require a reply, a login or a phone call.

We never add you to a mailing list because you booked an appointment. Care and marketing are separate.

How long we keep it, and how to have it deleted

Website inquiries are kept while they are useful to your care and then removed. Clinical records are kept for the period Georgia law requires, which is longer and not ours to shorten.

To see what we hold about you, correct it, or have it deleted, email admin@100yearlifestyle.com or call (770) 509‑9938. We will confirm your identity first — otherwise the process would itself be a privacy hole — and then act within thirty days.

Children

We treat children, and their parents book for them. This website is not directed at children and we do not knowingly collect information from a child through it. A parent or guardian completes the form.

Cookies

This website uses cookies to help personalize your online experience. A cookie is a text file placed on your hard disk by a web page server. Cookies cannot be used to run programs or deliver viruses to your computer, and they can only be read by a web server in the domain that issued the cookie to you.

One of the primary purposes of a cookie is to save you time — it tells the web server that you have returned to a specific page, so information you previously provided can be retrieved and the features you customized still work.

You have the ability to accept or decline cookies. Most web browsers accept them automatically, but you can usually change your browser setting to decline them. If you decline cookies, some interactive features of this website may not work fully.

Why we collect information, and for how long

To understand your needs and deliver the services you have requested; to improve our services; to send you email you have consented to receive; to invite you to surveys or research when you have consented; and to tailor the website to how it is actually used.

Data is stored no longer than necessary. How long that is depends on how long the information remains relevant to your care, the records we must keep to show we met our obligations, any limitation period in which a claim could be made, and any retention period required by Georgia or federal law — whichever is longest.

When information may be disclosed

Plasker Family & Performance Chiropractic does not now, and will not in the future, sell, rent or lease its patient or customer lists to any third party. Your information may be disclosed only in these circumstances:

  • As necessary to provide the services or products you have requested.
  • In other ways described in this policy, or to which you have otherwise consented.
  • Aggregated with other information so that your identity cannot reasonably be determined.
  • As required by law, or in response to a subpoena or search warrant.
  • To outside auditors who have agreed to keep the information confidential.
  • As necessary to enforce our terms of service.
  • As necessary to maintain, safeguard and preserve the rights and property of Plasker Family & Performance Chiropractic.

Sensitive information, and public posts

We do not use or disclose sensitive personal information such as race, religion or political affiliation without your explicit consent.

If you post personally identifiable information in a public place — a review, a social media comment, any public message board — that information can be collected and used by others. We do not read your private online communications.

We encourage you to review the privacy statement of any website you reach from a link here. We do not control those websites and are not responsible for their content or their practices.

Non-marketing contact

Plasker Family & Performance Chiropractic reserves the right to contact you for non-marketing reasons — a security issue, a problem with a form you submitted, an account or appointment issue, or a material change to this policy. In some circumstances we may post a notice on this website instead.

Children under the age of 13

This website is not directed to children under the age of 13 and does not knowingly collect personally identifiable information from them. If we determine such information has been collected inadvertently, we will delete it promptly or obtain verifiable parental consent for its use and storage. Anyone under 13 must have a parent or guardian's permission to use this website.

How your information is secured

Sensitive information submitted through this website is encrypted in transit — you can confirm this by the lock icon in your address bar and the “https” at the start of the address. We use Secure Socket Layer (SSL) for authentication and private communication.

Information is protected offline as well. Only staff who need it to do a specific job are granted access to personally identifiable information, and the computers and servers holding it are kept in a secure environment to prevent loss, misuse, unauthorized access, disclosure or modification.

Notice to European Union users

Plasker Family & Performance Chiropractic operates in the United States. If you provide information to us, it is transferred out of the European Union and sent to the United States. By providing personal information to us, you consent to its storage and use as described in this policy.

Acceptance of these terms

By using this website you accept the terms of this privacy policy. If you do not agree with it, please do not continue to use the site. Continued use after any update or change is posted means you accept that change.

Changes, and the honest caveat

If this policy changes materially we will change the effective date and, where the change affects email you have asked for, tell you in an email rather than quietly amend the page.
